You’ve received communication that your questionnaires have successfully been converted to FHQ4 and you are ready to check them out and/or Go Live using FHQ4.
Fun Facts
- FHQ3 and FHQ4 questionnaires will live concurrently in your database until we do an official end-of-life (EOL) of FHQ3. We will be in communication about this at a later date.
- All questionnaires you have sent to date have been FHQ3 and all those invitations in which have not been completed will be finished/completed in FHQ3. Automatic or manual reminders for these invites will be for completing the questionnaire in FHQ3.
- To minimize confusion with your patients, we suggest that you have your patients complete their questionnaire in FHQ3 versus sending them a new invitation for the FHQ4 version.
- When converted, the questionnaires have the same name as they were before + (FHQ4) amended to the end (i.e. Full Risk Assessment > Full Risk Assessment (FHQ4).
Testing your FHQ4 Questionnaires
Upon conversion completion, you were provided with the Generic Links of your FHQ4 version questionnaires. The Generic Link is a URL that can be used to start a new questionnaire from scratch and upon creation of new credentials (which can be faux credentials just for this testing) a new pedigree will be created.
Use the Generic URL(s) that you received to start a new questionnaire and see how it looks, feels, and functions. It’s even MOBILE-FRIENDLY, so try it out on your cell phone, too. Your content remained the same between FHQ3>4 conversion, but it has been converted into the new formatting of FHQ4.
Generic Links
Speaking of Generic Links. It’s possible you placed a Generic Link for one of your questionnaires in an email, on a website, bookmarked on your computer, on a printed documented that patients receive, etc. The Generic Link for your questionnaires have changed and need to be replaced with the new Generic Link(s) that you received upon conversion completion.
Going Live with FHQ4
All that is needed to “Go-Live” with FHQ4 is you and all users who send FHQ invitations need to decide a stop date that will be the last day that you send an FHQ3 invite and a start date that you will begin to send FHQ4 questionnaire invitations.
As mentioned previously, FHQ3 and FHQ4 questionnaires live in Progeny simultaneously, so there will be existing invites that patients will be finishing up in FHQ3 and you will be sending new questionnaires in FHQ4 and patients will be completing those in FHQ4.
The workflow in which you send invitations has not changed, you will just now see there are more questionnaire options listed and all FHQ4 versions will be appended with ‘(FHQ4)’ and they are labeled FHQ4 as the FHQ Version upon invitation.
