FHQ3>4 Conversions

FHQ administration can only be done by the ‘Progeny’ superuser or a user designated as ‘Administrator’

  • Click Invites>Edit Questionnaires
  • Find and select questionnaire to convert, click ‘Convert to FHQ4’ and ‘Yes’ in the prompt box
  • Close Edit Questionnaires module
  • Click Invites>Questionnaire Design
  • Find and select questionnaire converted (will be name of FHQ3 questionnaire with (FHQ4) appended to the end)
  • The following tabs/sections need a few modifications – see details about each of these sections on Learning Center.
  • Questionnaire Home
    • External URL: if empty, please input with the URL of your FHQ web server (i.e. https://www.hospitalhealthfhq.com)
  • Configuration
    • Just due to how these section headings and content were organized before and post FHQ4, there may be a need to modify the sections circled below to ensure these display as you would expect
  • Email & Reminders and Status Actions should not require any edits, everything should convert to look/work as expected
  • Questionnaire Designer
    • If you were familiar with how the questionnaire was designed page by page in FHQ3 using a datasheet format – this is now replaced with this section. If not familiar, this should bring a much more user-friendly functionality for editing questionnaire pages and content.
    • The conversion will bring over all pages that were present in your FHQ3 and will add all fields/questions to those pages. Each page is represented here and your pages and questions may need to be reworded after the conversion. See specific section in Learning Center on how to make edits to the pages and fields/questions. You will want to open each page (using pencil icon) and ensure that the page is named as you would like and that all fields/questions are worded and in the correct order as you would expect. You will notice there will be 1) fields/questions in wrong order and 2) fields/questions that are currently displaying the database field name and need to be update to the actual question patients should see. Both of these are unavoidable due to how FHQ3 questionnaires were previously configured.
    • The new to FHQ4 page, ‘Affected family members,’ will be there after the conversion but turned off – if you want to add this, see above Learning Center article and we would suggest you remove the field/question that you currently ask of all family members in the ‘Describe your family’ page (such as Cancer History?) and add that field to the ‘Affected family members’ page.
